About The Position

This is a grant-funded position primarily in the Arlington area, subject to the award and continuation of the contract. The Community Health Worker role is connected to system changes to address social determinants of health and facilitates access to health services for underserved community members. The role empowers individuals to be servant leaders in Community Health. Bilingual is preferred.

Requirements

  • H.S. Diploma or equivalent is required.
  • Six (6) months customer service experience either face-to-face or by phone is required.
  • BCLS – Basic Cardiac Life Support is required within 6 months of hire.
  • Strong verbal and written communication skills.
  • Ability to maintain composure in stressful situations.
  • Ability to problem solve.
  • Time management skills.
  • Detail-oriented.
  • Ability to work independently as well as with others.
  • Strong organizational & prioritization skills.
  • Ability to motivate and inspire others.

Nice To Haves

  • Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
  • Bi-lingual a plus.
  • Experience working with pregnant and postpartum moms.
  • Familiarity with prenatal care, postpartum care, Medicaid, uninsured, or underinsured populations.
  • Six (6) months of public health experience preferred.
  • Community health worker certification with the state of Texas is preferred.

Responsibilities

  • Responsible for establishing trusting relationships with patients and families.
  • Assist patients with personal goal setting and care plans.
  • Supports clients with meeting personal goals including but not limited to attending appointments, providing support for scheduling meetings/appointments with agencies and resource providers. Efforts will support development of self-management skills that improve health and well-being.
  • Collaborates with Senior Director of Faith/Community Health, regional Community Health Improvement initiatives and Community Health Ministry Strategy to implement Community Health Improvement activities.
  • Participate in program development, planning and evaluation.
  • Develop and effectively manage program timelines to ensure timely completion of program deliverables.
  • Complete required reports.
