As a Community Health Worker Workleader, you will provide leadership and training to assigned employees. In this role, you will oversee the daily operations of other Community Health Workers while also performing the duties of the assigned position. You will support and coach Community Health Workers in delivering a positive patient experience and working successfully within the Pathway Hub, perform quality reviews of monthly OKRs and performance, ensure achievement of metrics and support completion of EPIC documentation. A caregiver in this position works Monday through Friday from 8:30 a.m. – 5:00 p.m., with occasional community events, with advance notice and team rotation.

Requirements

  • High school diploma/GED
  • Community Health Worker (CHW) Certification through the Board of Nursing
  • Valid Driver’s license
  • Four years of work experience as a CHW (Community Health Worker) or patient navigation field
  • Demonstrated time management skills with the ability to balance multiple priorities and meet timelines
  • Ability to work effectively in stressful, sensitive and emergency situations
  • Ability to provide instructions and communicate effectively, both orally and in writing, with individuals of varying backgrounds and abilities
  • Ability to consider the role of cultural, social and behavioral factors in the accessibility, availability, acceptability and delivery of public health services
  • Ability to maintain accurate and complete records

Nice To Haves

  • Medical Assistant training or Medical Assistant certification
  • Bilingual in English and Spanish
  • Epic experience
  • Experience with electronic medical records (EMR)
  • Medical terminology knowledge
  • Experience as a CNA, Medical Assistant, or other patient-facing healthcare role
  • Community outreach or human services experience

Responsibilities

  • Provide cultural mediation among individuals, communities and health and social service systems, including education on navigating health and social service systems, building health literacy and supporting cross-cultural communication.
  • Under the supervision of professional staff, assist in creating a positive and supportive work environment, enforce a safe workplace and promote a culture of teamwork, communication and respect for living and working in a multicultural society.
  • Provide coaching and social support by motivating and encouraging individuals to obtain care and other services, supporting self-management, and promoting disease prevention and management of health conditions.
  • Advocate for individuals and communities by addressing their needs and perspectives, connecting them to resources and advocating for basic needs.
  • Build individual and community capacity by providing training and developing capacity among CHW peers and groups of Community Health Workers.
  • Develop and maintain working relationships with clients and make referrals to community agencies and services.
  • Participate in evaluation and research activities related to CHW services and programs.
  • Other duties as assigned.
