About The Position

The Position Incumbent works under the direction of the assigned Division Chief or designee, coordinating, directing, and managing the operations and services of the Community Health Worker (CHW) program. This role involves ensuring effective prioritization and assignment completion to meet health department goals and grant requirements, coordinating training for CHWs, and establishing effective work relationships with various stakeholders. The incumbent will facilitate training and peer mentoring, coordinate staff development, and oversee logistics for community health education and outreach events. A key aspect of the role is collaborating on the implementation of Social Determinants of Health (SDOH) screening tools to connect clients with essential non-medical resources and drafting strategies to support clients in navigating healthcare systems. The position also involves overseeing CHW documentation, analyzing outreach metrics, preparing reports, and maintaining job knowledge through continuing education. The role requires drafting standardized procedures, demonstrating empathy and respect, ensuring program readiness, maintaining confidentiality, providing guidance to subcontractors and staff, promoting a positive work environment, and participating in committees and emergency response planning. Safely operating a motor vehicle may be required for travel on County business.

Requirements

  • Bachelor’s degree in Public Health, Public Health Administration, Health Care Administration, Social Work, or Management required.
  • Two (2) years of MSOffice experience required.
  • Valid driver’s license and own transportation upon hire and maintained throughout employment required.
  • Completion of a pre-employment criminal record check and conditional offer drug screen required.
  • Equivalent combination of education and work experience may be substituted for requirements on a year-for-year basis.

Nice To Haves

  • Two (2) years as a Community Health Worker or experience in community health outreach position preferred.
  • One (1) year of supervisory experience preferred.

Responsibilities

  • Coordinate and monitor program activities, ensuring effective prioritization and assignment completion meeting health department goals and grant requirements.
  • Coordinate, record, and maintain training activities for current and new CHWs.
  • Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
  • Comply with Civil Service Rules, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.
  • Facilitate training and peer mentoring for CHW staff, focusing on core competencies, professional boundaries, and career development.
  • Coordinate staff development needs and recommend appropriate professional development opportunities for CHWs to the Supervisor.
  • Coordinate logistics for community health education activities and outreach events.
  • Collaborate with the Supervisor implementing Social Determinants of Health (SDOH) screening tools, ensuring clients connections to essential non-medical resources (e.g., housing, food, transportation).
  • Draft and recommend strategies supporting clients in navigating healthcare systems and advocating for health needs.
  • Oversee CHW documentation, analyze outreach metrics identifying service gaps and report on program impact.
  • Prepare and maintain various records, reports, correspondence and documents.
  • Maintain job knowledge and skills (e.g. seminars, webinars, conferences, research, meetings, continuing education).
  • Respond to questions and inquiries within expertise.
  • Draft and recommend standardized procedures and program policies streamlining CHW workflows and enhancing consistency of community outreach services.
  • Demonstrate empathy, respect and understanding toward individuals/families and community values.
  • Ensure program readiness assigning outreach duties, monitoring field activities, and conducting meetings for program updates.
  • Maintain confidentiality of medical and personal health information.
  • Provide guidance and technical assistance to subcontractors and staff.
  • Promote positive work environment and assist in resolving conflicts.
  • Participate in internal and community committees.
  • Participate in public health emergency response planning and respond during emergencies.
  • Safely operate a motor vehicle when required to travel on County business.
  • Report to work with regular, consistent attendance.
  • Perform other duties as assigned and directed.
