About The Position

The CHW - Patient Navigator will act as part of the patient care team by implementing clinic-based programs that address patient needs related to social determinants of health, as well as patient in-reach initiatives and campaigns. This role will learn and obtain knowledge around the resources and culture of the local community to better understand the needs of patients and have a positive impact on the overall patient experience and health outcomes.

Requirements

• Associate’s degree. Experience in a health care setting may be substituted year for year for the education requirement.
• One year experience in a health care setting, community health education and/or outreach position.
• Fluency in English is required: additional fluency in another language is valued.
• Ability to travel in the Seattle area as needed.
• If using personal vehicle to travel,valid Driver's License and Proof of Insurance is required.
