About The Position

We are currently seeking a Community Health Worker - Outreach who will utilize personal experience and training to assist individuals and families accessing, engaging, and utilizing community resources including health and wellness services. They will assist with new and existing clients/patients in solutioning areas of needs leading to improved health outcomes. Working with UCFS team members, they will assist individuals in meeting health goals and decreasing health disparity by increasing access to care.

Requirements

  • Associate in Human Services or related field
  • 1 year of experience working with community resources
  • 1 year of experience in the generation of care plans for clients or patients
  • Community Health Worker Certification
  • Ability to speak fluently in Spanish or Haitian Creole
  • Ability to travel 25 - 30% of a work schedule
  • Familiarity with software programs such as Microsoft Office Suite, especially in Outlook, Teams, and Excel
  • Experience with electronic health record systems such as EPIC
  • Ability to provide a strength based, client centered approach to the delivery of service.
  • Demonstrated excellent communication and interpersonal skills
  • Ability to work collaboratively in a team environment or individually as needed

Responsibilities

  • Provides initial and on-going tele-phonic and in person support to clients/patients seeking assessment for social determinants of health (SDOH)’s such as basic needs; legal, medical, housing, transportation, insurance by offering and completing a comprehensive needs assessment.
  • Collaborates with the patient/client and behavior health, primary care, dental and other UCFS services to develop and implement a patient centered care plan that supports client empowerment and improvement of overall health status.
  • Provides peer mentoring, coaching, skill building and guidance relevant to consumer needs (social skills, recovery supports, training, health awareness and recreational activities, etc.).
  • Establishes and facilitates a client/patient communication plan, timeline and follow up.
  • Maintains familiarity with community resources enabling connection to services for clients/patients and collaborate with state and local agencies and other community-based supports.
  • Conducts outreach in the community to targeted groups and individuals to identify and address barriers to accessing and utilization of health and social services.
  • Performs related administrative duties, including documentation of care coordination activities and any other record-keeping and administrative tasks required for reporting purposes.
  • Organizes and implements activities inclusive of awareness events, speaking engagements, dissemination of written materials.
