About The Position
The main purpose of this position is to perform at the full performance level of work providing health education, disease prevention information, SDOH screenings and resource navigation to medically underserved populations within the community. Operating within the Outreach and Education Division, this position supports the agency's mission by promoting healthy behaviors, reducing health disparities, and bridging gaps between vulnerable community members and critical public health services.
Requirements
- Graduation from an accredited high school or possession of a high school equivalency certificate.
- One year of experience in providing information on health care and disease prevention within the community.
- Possession of a motor vehicle operator's license valid in the State of Maryland (if assigned duties requiring vehicle operation).
Nice To Haves
- An active Certified Community Health Worker (CCHW) credential in the State of Maryland.
- Experience linking residents to local resources.
