The Greater Lawrence Family Health Center (GLFHC) is seeking a Community Health Worker / Medical Assistant (CHW/MA) for the Moms Do Care (MDC) program. This role is part of an integrated team focused on addressing the clinical and social needs of obstetric (OB), post-partum, and parenting patients with Substance Use Disorder (SUD). The CHW/MA will utilize knowledge of community resources and culture to assist the OB and SUD team in assessing patient needs and developing comprehensive care plans. This position supports the MDC Registered Nurse (RN) by performing nursing support, clerical tasks, case management, outreach, education, social support, and linkages to appropriate services through coordination and navigation. The role requires the ability to wear appropriate personal protective equipment (PPE), perform accurate vital sign assessments, document in the Electronic Medical Record (EMR), and participate in intakes and assessments. The CHW/MA will assist the RN in developing care plans with a focus on social determinants of health, link patients to Behavioral Health services and community resources, and provide short-term follow-up. Responsibilities include coordinating appointments, ensuring timely treatment and navigation, tracking medical and behavioral appointments, and developing appointment reminder strategies. The CHW/MA must stay current on referral sources for housing, nutrition, peer programs, Mass Health/Healthy Start, advocacy, transportation, parenting skills, prenatal classes, substance abuse and mental health services, domestic violence, employment, nursing case management, and primary care.
