About The Position

Home & Health Care Management is the oldest home health care agency in Northern California, specializing in in-home care and case management for older adults, children, and individuals with disabilities. The Community Health Worker (CHW) Lead supports the CHW Program Manager in the training, development, implementation, and operational performance of the CHW, Enhanced Care Management (ECM), and Community Supports (CS) programs. This role assists in coordinating and supporting program workflows, staff activities, and member services in alignment with organizational standards and regulations. The CHW Lead provides day-to-day operational guidance to Community Health Workers, Care Managers, and Housing Navigators, focusing on workflow execution, documentation quality, and team communication. The role assists in monitoring program performance, identifying operational or compliance concerns, and escalating issues to the Program Manager. The CHW Lead collaborates with internal departments to support program quality, efficiency, and growth, functioning in a supportive leadership capacity under the direction of the Program Manager.

Requirements

  • High school diploma or higher education
  • Skilled in operating a personal computer utilizing a variety of standard software
  • Ability to work independently and as a member of a multidisciplinary team
  • Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • CPR certificate required within 30 days of hire

Nice To Haves

  • Associate’s degree or higher in Public Health, Social Work, or a related health or social services field preferred; equivalent relevant experience may be considered.
  • Working knowledge of state and federal program requirements related to community-based services, care coordination, aging, and disability populations.
  • Knowledge of Medi-Cal, Enhanced Care Management, Community Supports, and Community Health Worker program requirements and documentation expectations.
  • Prior experience in a lead, senior, or supervisory support role preferred.
  • Bilingual or multilingual skills reflective of the population served.

Responsibilities

  • Designs, conducts, and monitors the onboarding process for new staff, including orientation, training, workflow integration, organizational goals, and caseload transition.
  • Travels to Chico for a designated period to conduct training for new Chico staff.
  • Accepts overtime when necessary.
  • Prioritizes competing tasks, manages multiple deadlines, and maintains timely follow-through on assignments and appointments.
  • Maintains confidentiality in accordance with HIPAA and organizational policies.
  • Monitors care management team workflows for consistency and accuracy, reviews caseload documentation for completeness, and escalates concerns to the Program Manager.
  • Assists Program Manager in monitoring and tracking ECM, CS, and CHW participants and maintaining an accurate program census.
  • Conducts preliminary documentation and case file reviews to identify trends, gaps, or areas for improvement and reports findings to the Program Manager.
  • Assists Program Manager in coordinating and planning outreach strategies and participates in outreach events and community engagement activities.
  • Travels within the service area to participate in community meetings or corporate meetings.
  • In the absence of the Program Manager, processes time sheets for staff and performs QA on submitted timecards.
  • Assists in staff performance management, including coaching and participation in counseling discussions, as directed by the Program Manager or direct supervisor.
  • Performs other duties as required and assigned by Program Manager.
