About The Position

The Community Health Worker (CHW) will be an integral part of a multidisciplinary team, primarily supporting referrals to the Caring Dads program. Key responsibilities include engaging Fathers, managing intake documentation, conducting Social Determinants of Health screenings, and utilizing other screening tools to identify areas of need. The CHW is responsible for ensuring that Fathers are referred to appropriately matched services. Additionally, the role involves co-facilitating a specialized 17-week group intervention for referred Fathers, focusing on promoting healthy family interaction, active listening, child-centered parenting, and fostering non-abusive, co-parenting relationships. The CHW will also act as a "Mother Contact," connecting with identified partners to discuss program components, complete necessary Social Determinants of Health screenings, support care coordination to matched services, and provide ongoing support and monitoring.

Requirements

  • High School Diploma or Equivalent
  • Ability to meet all requirements to be certified as a Community Health Worker within the first 12 months of employment
  • Strong interpersonal and social skills with an ability to collaborate with a variety of community based organizations, coalitions and stakeholders
  • Commitment and sensitivity to issues related to the needs of parents and children, as survivors of Domestic Violence
  • Ability to work independently and as an interdependent team member
  • Ability to provide services at agency sites, community locations, and virtually as required
  • Ability to have a flexible schedule to include some evenings
  • Must pass RI BCI, National Fingerprint & DCYF CANTS check
  • Valid driver’s license and reliable transportation

Nice To Haves

  • Associates or Bachelor’s degree
  • Community Health Worker Certification
  • Bilingual in Spanish or Portuguese
  • Prior experience in group facilitation, victim advocacy, domestic violence offenders and/or crisis intervention
