About The Position

The CSW plays a vital role in engaging and supporting patients in the SUD program, with a person-centered, culturally responsive approach. The CSW builds trusting relationships with patients and serves as a bridge between the patient, the addiction specialists and the medical providers. This position supports connects patients who are already in treatment, provides outreach internally and externally to potential participants and connects patients to internal and external resources.

Requirements

  • Certification or training as a Community Health Worker, peer advocate, or related paraprofessional role preferred; BA level candidate with knowledge of addiction may be considered in lieu of CSW certification
  • Previous experience in a healthcare, social services, or community-based setting preferred.
  • Ability to work as part of a team in a respectful and non judgmental manner
  • Demonstrated commitment to community engagement and supporting underserved populations.
  • Ability to model positive health behaviors and support both a recovery approach
  • Good interpersonal, and advocacy skills to effectively engage with members, families, healthcare providers, and community organizations.
  • Ability to build rapport, trust, and supportive relationships with patients from diverse cultural and socioeconomic backgrounds.
  • Skilled in communication and active listening
  • Familiarity with local community resources, public assistance programs, and social service systems.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Strong organizational skills, with the ability to manage multiple patients and tasks.
  • Comfort with learning documentation, data entry, and use of electronic health records or case management systems.
  • Commitment to cultural humility and patient-centered engagement.
  • Ability to find solutions when barriers are identified.
  • Good documentation skills.
  • Ability to multi-task and prioritize when needed.
  • Ability to independently seek out resources and work collaboratively.
  • Ability to read, understand and follow oral and written instructions.
  • Demonstrates a willingness and ability to work under supervision.
  • Ability to develop and maintain good working relationships with staff.
  • Ability to use computer and learn new software programs.

  • Demonstrate flexibility, enthusiasm, and willingness to cooperate while working with others in multi-disciplinary teams.
  • Performs other related duties as assigned.

Responsibilities

  • Conducts internal and community outreach activities to identify patients who might benefit from SUD services
  • Works closely with medical, dental and behavioral staff to identify potential patients
  • Facilitates navigation throughout CCHC and as well as community resources to ensure coordinated care and follow up
  • Utilizes EHR to coordinate services and input data for reporting.
  • Supports current patients in the SUD program in their recovery journey and encourages continued participation
  • May provide intake and scheduling activities, as needed
