About The Position

Working out in the community, the Outreach and Eligibility Worker determines the eligibility of applicants and recipients for public assistance programs through interactive interviewing and fact gathering. They maintain current knowledge of program regulations and procedures necessary for multi-program caseload administration, and initiate and process casework through an automated system. This role identifies needs and makes appropriate referrals for health, social, and/or employment services. The position serves as a liaison between BACH and community groups as well as other non-profit agencies. Assists in follow-up with patients who are eligible for health care programs at BACH. Develops, implements, and translates bi-lingual health education materials. Conducts bi-lingual presentations on BACH programs and services to community agencies. Under the general direction of the Outreach Supervisor, the Health Services Advocate performs a variety of complex, professional, analytical, and confidential Outreach services. Provides outreach activities at BACH by assisting current and potential patients to gain access to health care coverage and BACH services provided by the clinic, including health screenings, immunization and parent education. Serves as a liaison between BACH and community groups as well as other non-profit agencies. Demonstrates commitment to, and understanding of, BACH’s Behaviors for Success and GUEST (Customer Service Model), by modeling customer service excellence in all internal and external relationships and in the performance of all duties and responsibilities. Develops, implements, and translates bi-lingual health education materials. Conducts bi-lingual presentations on BACH programs and services to community agencies, as needed. Collaborates with other departments and providers in all aspects that contribute to marketing, outreach, and education of BACH’s services. Maintains accurate records and submit program reports as required. Prepares informational and marketing materials for BACH. Participates in health fairs, presentations and community outreach events. Assists in the development and implementing of outreach strategies. Assists in development of grant/contract resources to support expanded health care programs. Conduct all work activities in compliance with applicable laws, regulations, statutes, and all BACH policies and procedures; immediately reporting all observed or suspected non-compliant behavior to your Supervisor/Manager, the Compliance Manager, and/or the confidential reporting hotline. Works flexible schedule Monday through Sunday to include day and evening hours.

Requirements

  • High school diploma or equivalent education and experience.
  • Six month’s customer service experience with the demonstrated ability to work with people of diverse backgrounds with respect to race, ethnicity, class, sexual orientation and disability.
  • Experience conducting outreach activities and presentations.
  • Bilingual; Mandarin, Spanish, Punjabi.
  • Self-starter, able to work independently.
  • Demonstrated ability to show non-judgmental attitude and maintain client confidentiality.
  • Access to reliable transportation.
  • Ability to work flexible schedule seven days a week including evening hours.
  • Proficient in Microsoft and Internet.
  • The ability to work and act independently, and to make and carry out decisions with minimal supervision.
  • The ability to communicate effectively, both orally and in writing.
  • Strong interpersonal skills with the ability to lead teams and projects, and work collaboratively.
  • Maintaining confidentiality and HIPAA compliance.
  • Excellent organizational, time management and multi-tasking skills with attention to detail.

Nice To Haves

  • Current knowledge of Medi-Cal, Healthy Families, Alameda County Health PAC and other health care coverage programs.
  • Six months medical eligibility experience, experience related to the duties and responsibilities.
  • One-year experience in health or human service agency.
  • Knowledge of California Health Plans, County Programs, and the Affordable Care Act (ACA).
  • Certified Enrollment Counselor (CEC) Certification for Covered California.
  • Experience with Electronic Health Records (EHR).
