The Community Health Specialist will play a key role in expanding the reach and accessibility of the products and tools related to the Association’s Rural Health initiative designated with each state. This position works with the state Community Health Manager and is responsible for identifying, engaging, and onboarding local organizations, health care providers, public health organizations, community health workers, individuals and health systems, including community health centers, rural health clinics, hospitals, and specialty practices to ensure smooth access and adoption of the key resources and tools towards the deliverables of the state specific initiative. Working with other local and state Association staff, the Specialist will guide providers through program onboarding, connect users to training, answer questions, and support ongoing participation of any number of initiatives and services across brain health, care navigation services and supports and workforce enhancement. They will share updates and insights on program performance, impact, and opportunities for improvement as well as support reporting structures. Through proactive communication and support, the Specialist will help seamlessly integrate the rural health initiatives into users' processes and supports to enhance dementia care services in rural settings. This position is grant funded.
