About The Position

Under the direction of the Behavioral Health Office Manager, the Community Health Representative acts as a representative, transporter, and courier for the behavioral health program. This role involves conducting home visits, serving as an advocate, and facilitating problem resolution for the Native American Indian community in obtaining needed medical care. The position requires strong communication skills, the ability to build rapport with patients and families, and sensitivity to the cultures, traditions, and values of the Indian community.

Requirements

  • Ability to maintain accurate records and prepare clear, accurate reports.
  • Attention to detail and ability to review and verify data accuracy.
  • Ability to organize and follow one's own work schedule, including setting priorities, being available during work hours, and meeting deadlines.
  • Knowledge of medical terminology.
  • Competency and integrity to maintain the confidentiality of organizational, employee, and client records.
  • A working home/cell phone for immediate contact.

  • A high school diploma or general education degree (GED).
  • 2 years or more of related experience and/or training, or an equivalent combination of education and experience.
  • A current California driver’s license and insurability with the Tule River Indian Health Center, Inc. insurance agency.
  • Valid First Aid/CPR certification at the BCLS level or ability to obtain within 3 months of hire.

Nice To Haves

  • One (1) year prior experience in a health care setting.
  • Familiarity with entering data into a healthcare database program (EHR).
  • Certified Medical Assistant and/or Certified Nursing Assistant.
  • Ability to obtain CHR certification through IHS within 6 months of employment.

Responsibilities

  • Conduct home visits to members of the Native American Indian community within the service area according to established policies and care plans.
  • Provide limited courier services for medication delivery, clinic messages, and transportation of clients to appointments.
  • Offer advocacy and education services to clients, facilitating access to health services and assisting with appointment scheduling.
  • Perform limited Home Health Aide-type services for seniors, at-risk adults, and ill or handicapped patients/clients.
  • Maintain reporting requirements, including documentation of activities, patient logs, and other forms for administrative, billing, and quality assurance purposes.
  • Collaborate with other health program staff and assist in other departments to support a team approach to healthcare delivery.
  • Participate in community events as needed in collaboration with the Behavioral Health Department.
  • Provide First Aid/CPR (BCLS level) to patients/clients as necessary.
  • Maintain vehicle logs and maintenance schedules, coordinating with the Transportation Department for vehicle safety and maintenance.
  • Work flexible hours as required for medical/dental appointments, distant transports, and community awareness events.
  • Drive long distances and navigate large metropolitan areas confidently.
