About The Position

The Community Health Representative promotes Family HealthCare Network services through outreach activities, coordination of community events, and participation in relevant grant initiatives and special programs. This role involves carrying out responsibilities in community outreach, enrollment assistance, coordination of FHCN community events, grants, and programs, and achieving departmental goals and metrics. The representative will communicate feedback to their supervisor, serve as a subject matter expert, and perform high-level administrative functions to comply with regulations and reporting requirements. They will execute outreach activities, participate in community events to disseminate information and provide health screenings, and lead community focus groups and surveys. The role also includes executing network-hosted community events, conducting presentations and educational workshops, and maintaining relationships with strategic partners. Additionally, the representative will act as a liaison between FHCN and patients, support consumers with enrollment services and referrals, conduct intake and screenings for various programs, educate consumers on health coverage, and provide ongoing case management. They must complete and maintain all obligatory certifications, stay abreast of program updates, and execute FHCN programs and departmental grant initiatives, including implementing program and grant curriculums. Daily operations at food pantries, including inventory and receiving deliveries, are also part of the responsibilities. Compliance with all program or grant-related training and certifications, and maintaining professional working relationships with partners and funders are essential. Adherence to the Attendance and Absenteeism Policy and the ability to travel between FHCN locations and other non-FHCN locations are required. Other duties as assigned.

Requirements

  • Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, technical, or health care-related knowledge frequently acquired through completion of a trade school, para-professional, or certificate type program.
  • Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to create basic presentations in Microsoft PowerPoint.
  • Valid CA driver’s license and car insurance.
  • As a condition of employment at Family HealthCare Network, all employees assigned to provide Covered California education and enrollments must obtain certification as a Certified Enrollment Counselor. This will include successfully completing state-mandated training, passing state certification exams, and passing a Federal Background Check.
  • Any staff assigned to support food programs must obtain a food handlers certification upon hire.
  • Job duties require compiling information in effective written form, including correspondence, reports, articles, or other documentation.
  • Effectively conveys technical information to non-technical audiences.
  • The employee is regularly required to sit and use repetitive hand movements to type and grasp.
  • The employee is frequently required to stand or walk.
  • The employee must occasionally stoop, reach above and below shoulder height, squat, climb, lift, and/or move up to 30 pounds.
  • The employee must work in the environmental elements, including cold and hot weather.
  • The employee must travel from site to site and to other non-FHCN locations.

Responsibilities

  • Carrying out responsibilities in community outreach, enrollment assistance, coordination of FHCN community events, grants, and programs.

  • Maintaining relationships with strategic partners and participating in mutually beneficial activities.
  • Acting as a liaison between FHCN and patients.
  • Supporting consumers with enrollment services and referrals.
  • Conducting intake and screenings to determine eligibility for Medi-Cal, Medicare, Covered CA, CalFresh, sliding fee, food pantries, and literacy programs.
  • Educating consumers on health coverage benefits and rights and assisting with navigating services.
  • Providing ongoing case management to ensure healthcare coverage retention and utilization of FHCN services.
  • Completing and maintaining all obligatory certifications as required.
  • Staying abreast of State and local program updates.
  • Executing FHCN programs and departmental grant initiatives.
  • Implementing programs and grant curriculums at sites and in community settings.
  • Executing day-to-day operations at food pantries and recording daily inventory of food items and related materials.
  • Receiving product deliveries and ensuring items are refrigerated and stored appropriately.
  • Participating in events to meet program and grant objectives.
  • Ensuring compliance with all program or grant-related training and certifications.
  • Maintaining a professional working relationship with all program partners and grant funders.
  • Adhering to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Performing other duties as assigned.
