The Community Health Planner (CHP) promotes individual and population health through health promotion and education initiatives and health behavior-change programs in a variety of settings for the diverse populations represented in Delaware County. Responsibilities include developing, implementing, managing, and evaluating programs and public health initiatives, establishing, leading, and maintaining community partnerships, and managing work plans and timelines. CHPs may be focused on emergency preparedness, and will work with other County, Regional, State, and Federal agencies to coordinate, communicate, and execute public health preparedness activities, including enhancing strategies for mitigation, preparedness, response, and recovery. They will also provide resources and assistance to county residents and community organizations when they are faced with foreseen and unforeseen circumstances because of a disaster. Work involvement will include initiatives and projects that may change as the initiatives and projects evolve. This position will implement grant objectives, work plans and respond to priorities set forth by DCHD and local coalitions.
