About The Position

The Community Health Educator promotes and improves the health and well‑being of underserved individuals and families through culturally responsive education, outreach, and community engagement. This role empowers community members with the knowledge, skills, and resources needed to make informed health decisions and adopt healthy behaviors. Working collaboratively with care teams and community partners, the Community Health Educator delivers health education programs, preventive wellness initiatives, and resource‑navigation support. The role assesses social and behavioral needs, identifies barriers to care—including Social Determinants of Health (SDOH) such as housing, food security, transportation, utilities, education, childcare, and financial strain—and connects families to appropriate healthcare and community‑based services. Serving as a trusted liaison among families, healthcare providers, schools, and community organizations, the Community Health Educator enhances access to care, supports positive health outcomes, and contributes to the development of sustainable, community‑based health programming.

Requirements

  • High school diploma required.
  • 6 months Experience in community outreach, education, or direct service work with families.
  • Community Health Worker (CHW) certification.
  • Reliable form of transportation.
  • Ability to read, write, and communicate in English.
  • Bi-lingual (Spanish) required.

Nice To Haves

  • Associate’s degree in Community Health, Human Services, Family Studies or related social services field preferred.
