About The Position

Foodsmart is seeking a Community Health Outreach Coordinator to promote their foodcare program in partnership with Santa Clara Family Health Plan (SCFHP). This part-time contractual position is crucial for increasing enrollment in the Foodsmart program by identifying community partners and events, and developing strategies for enrollment through onsite activations and referrals. The ideal candidate will have strong interpersonal skills, extensive knowledge of the Santa Clara County community, and a passion for food as medicine. Experience navigating diverse Silicon Valley communities, including Latinx, Vietnamese, Filipino, and South Asian populations, is highly desirable. This role requires an on-the-ground presence to connect SCFHP members with Foodsmart's nutrition support services. The position is part-time, with up to 25 working hours per week, and requires travel throughout Santa Clara County and the surrounding Bay Area.

Requirements

  • Bilingual fluency in English and Spanish (required).
  • Ability to travel to work sites throughout Santa Clara County and surrounding areas (required).
  • Strong organizational skills and the capacity to maintain detailed tracking of event enrollment, referral activity, and partnership outcomes.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
  • Comfortable learning new platforms (e.g., ReferralMD, Salesforce, or similar CRM tools).
  • Willing and able to travel to community events, health centers, and FQHC sites throughout Santa Clara County.
  • Comfortable working in situations that involve confidential and sensitive member information.
  • Skilled at building effective working relationships with colleagues, senior staff, clinical partners, and community members.

Nice To Haves

  • Based within Santa Clara County.
  • Experience working with Medi-Cal populations, managed care plans, or community health programs in the Bay Area.
  • Familiarity with the Santa Clara County health ecosystem (e.g., Valley Medical Center, Santa Clara County Public Health, FQHCs, CBOs).
  • Proactive and community-rooted, acting as an advocate for the populations served.
  • The flexibility to handle additional duties as assigned and adapt to evolving program priorities.

Responsibilities

  • Identify active Community-Based Organizations (CBOs), Faith-Based Organizations (FBOs), Federally Qualified Health Centers (FQHCs), and providers serving the Santa Clara Family Health Plan member population.
  • Identify and evaluate local community events, health fairs, and clinic-based opportunities for onsite Foodsmart presence.
  • Attend enrollment events to raise awareness of Foodsmart's foodcare program and SCFHP benefits.
  • Cultivate relationships with identified organizations to build awareness of Foodsmart.
  • Define the scope of partnerships, including onsite events and referral pathways.
  • Collaborate with the Foodsmart marketing team to create event materials in various languages.
  • Educate diverse and underserved populations about healthy eating, chronic disease prevention and management, and SNAP/CalFresh benefits.
  • Screen event attendees for Foodsmart eligibility and schedule nutrition coaching appointments.
  • Motivate and encourage eligible members to complete nutrition programming and screening assessments.
  • Maintain a comprehensive activity log tracking events, referrals, appointments, and enrollment outcomes.
  • Attend bi-weekly team meetings and calls with internal Foodsmart and SCFHP client teams.
  • Support the CHC referral activation strategy, including outreach to FQHCs and CBOs.
