Temp Per Diem Community Health Nurse - LHCSA
About The Position
Provides skilled nursing care to patients in their home, including nursing assessment, interventions and supervision of Home Health Aides and Personal Care Aides in the field on a per visit basis. This is a per diem RN Field Supervisor position available with a well-established Licensed Home Care agency in Nassau County. Duties include Patient Assessment, Reassessment and Aide supervision for patients receiving personal care and NHTD services. Perform in-home client assessments and/or RN supervisory visits for home care aides. Complete and submit all agency paperwork in a timely manner regarding visits to meet NYSDOH requirements. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers. Adheres to the organization’s policy in regards to absenteeism and appearance.
Requirements
- Graduate of an accredited school of nursing
- One year of hands on nursing experience
- Must have current license and registration to practice as a Registered Professional Nurse in New York State
- Problem solving abilities while multi-tasking
- Ability to exercise sound judgment
- Excellent verbal and written communication skills
- Excellent customer service and organizational skills
- Must have a car and be willing to drive to Long Island client homes
Nice To Haves
- Bachelor’s degree (B.A.) in Nursing from a four-year college or university preferred
- One year Home Care experience preferred
- Experience with a Clinical EMR
- HHA Exchange experience preferred
