About The Position

Provides skilled nursing care to patients in their home, including nursing assessment, interventions and supervision of Home Health Aides and Personal Care Aides in the field on a per visit basis. This is a per diem RN Field Supervisor position available with a well-established Licensed Home Care agency in Nassau County. Duties include Patient Assessment, Reassessment and Aide supervision for patients receiving personal care and NHTD services.

Requirements

• Graduate of an accredited school of nursing
• Must have current license and registration to practice as a Registered Professional Nurse in New York State.
• Problem solving abilities while multi-tasking; ability to exercise sound judgment
• Excellent verbal and written communication skills
• Excellent customer service and organizational skills
• Must have a car and be willing to drive to Long Island client homes

Nice To Haves

• Bachelor’s degree (B.A.) in Nursing from a four-year college or university preferred.
• One year of hands on nursing experience.
• One year Home Care experience preferred.
• Experience with a Clinical EMR, HHA Exchange experience preferred

Responsibilities

• Perform in-home client assessments and/or RN supervisory visits for home care aides
• Complete and submit all agency paperwork in a timely manner regarding visits to meet NYSDOH requirements

• Adheres to the organization’s policy in regards to absenteeism and appearance
