The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information /education, referral and evaluation. Requirements

- Possession of, or ability to obtain, an appropriate and valid license to practice as a Registered Nurse issued by the State of Oregon.
- A Bachelor's degree in nursing from an accredited nursing program; OR An Associate's degree in nursing from an accredited nursing program AND two years of registered nurse level experience is required.
- Spanish Language Required
- BLS/CPR certification required
- Ability to work independently

Responsibilities
- Proactively assess, plan, implement and evaluate healthcare services that improves and promotes health outcomes.
- Provide regular screening, comprehensive assessment and interventions for on-going health issues (e.g. depression, diabetes, asthma, wound and pain management, and other chronic diseases).
- Assure completion of referrals and coordinate care with other healthcare providers.
- Collaborate with the healthcare team to adapt the plan of care.
- Assess clients needs for in-person and telephone response for urgent and non-urgent medical information.
- Provide health education and promote self management.
- Respond to emergency events as needed.
- Administer medical treatments and medications.
- Assess, diagnose and prescribe treatment according to standing orders and established protocols.
- Act as a resource for team needs.
- Maintain clinical skills as dictated by the client population and the clinical practice (adult, pediatric, OB, internal medicine).
- Review cases with the providers and team as needed.
- Seek expert advice on issues for clients and provide input to providers and nurses on client response.
- Completes competencies required for the position.
- Complete documentation and charting as required by policy
- Mentor, coach and offer clinical expertise to team members.
- Delegate the care of individuals and groups of clients to members of the team based on the scope of practice and clinical skills needed by the clients.
- Assure appropriate client follow-up.
- Foster community partnerships to best meet the needs of the client
- Participate and support the quality improvement process through team, role, and designated specialty workgroups.
