About The Position

The Community Health Nurse role with the Department of Health allows you to take part in disease prevention, education, and direct public health services. You will work closely with partners, monitor health trends, and help improve outcomes across the region. This position supports statewide public health goals by assessing needs, guiding initiatives, and providing direct nursing services. The role helps strengthen community programs, prevent disease, and promote healthier outcomes for residents.

Requirements

• Active authorization to practice as a Registered Nurse in Pennsylvania
• Must meet the PA residency requirement
• Must be able to perform essential job functions
• Must pass a background investigation
• Submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Nice To Haves

• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing

Responsibilities

• Review community data to identify public health needs and guide planning activities
• Link department resources with community partners to support local health initiatives
• Track communicable diseases, enter surveillance data, and support control measures
• Provide instruction on public health topics to support awareness and prevention efforts
• Deliver immunizations, testing, and related services according to established guidelines
• Offer case management, guidance, and support to individuals and targeted populations
