About The Position
Provide community-based nursing care to individuals and families, promoting health, managing conditions, and connecting patients to resources through assessment, education, and coordinated follow-up. End of life care experience needed.
Requirements
- Registered nurse with valid license
- Community nursing or primary care experience
- Knowledge of infection control and patient safety
- Ability to work independently and in teams
- Patient assessment and care planning
- Wound care and medication administration
- Chronic disease management
- Infection prevention and control
- Documentation in EHR/EMR
- Empathy and patient-centered communication
- Critical thinking and clinical judgment
- Time management and prioritization
- Collaboration with multidisciplinary teams
- Respect for cultural and individual needs

Responsibilities
