About The Position

Reema Health is seeking a Mandarin-speaking Community Health Navigator to join their Outreach & Engagement team. This role involves conducting phone outreach, enrolling members in Reema's care model, and managing a caseload of 80-120 members. The Navigator will assess member needs, including social determinants of health, to create personalized care plans. A key aspect of the role is building trusting relationships, supporting members in understanding health activities, overcoming barriers to care, and coordinating referrals and appointments. The position requires independent organization and prioritization to meet goals, proactive engagement through various channels, and connecting members to community resources, potentially including transportation. Maintaining professional boundaries, confidentiality, and thorough documentation in the Reema App are essential. The role also involves representing Reema Health positively, working independently and collaboratively in a fast-paced startup environment, and adapting to change. Requirements

  • Tech Literacy — comfortable learning and troubleshooting new tools on the fly, on both mobile and desktop.

  • Collaboration & Team Building — dedicated to fostering team connection in a remote environment, including participation in occasional required in-person team meetings.
  • Fluency in Mandarin, speaking and written.
  • 18 years of age or older.
  • Available to work Monday–Friday during core business hours.
  • Offers contingent upon passing a pre-employment drug screening and background check.
  • Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA.
  • Use technology-based tools and systems daily.
  • Lift and carry up to 25 lbs occasionally.

Nice To Haves

  • 1+ years of direct experience with the population/community served, or equivalent.
  • Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent direct experience).
  • Experience with crisis planning, case management, and professional boundaries in community-based work.
  • Relationship-building, community resource knowledge, and health system navigation.
  • Comfort working as part of a multidisciplinary team.

Responsibilities

  • Conduct phone outreach and enroll members in Reema's care model.
  • Build and manage a caseload (80-120 members), providing coverage for other Guides during absences or workload spikes.
  • Assess member needs, including social determinants of health, to build a member-centered care plan.
  • Establish trusting relationships and reliably follow through in a timely, appropriate manner.
  • Help members understand why a health activity matters, work through hesitation with empathy, and follow through to completion.
  • Help members steadily overcome barriers to care, with visible progress every month before the program ends.
  • Coordinate referrals and support appointment scheduling and attendance.
  • Independently organize and prioritize your workload to hit monthly goals and daily metrics.
  • Proactively engage with members using a comprehensive mix of outreach channels, including phone, text, and email.
  • Connect members to community and health resources, including transporting or accompanying them as needed.
  • Maintain professional boundaries and confidentiality; consult the multidisciplinary team on complex situations.
  • Document all member work thoroughly and on time in the Reema App, including keeping care plans current.
  • Comply with HIPAA and protected health information confidentiality standards.
  • Serve members with dignity and respect; build and maintain relationships with community resources.
  • Work independently and collaboratively in a fast-paced startup environment, seeing frequent change as an opportunity, not a barrier.
