What you’ll do Position Summary The Community Health Manager is a working manager responsible for both supervising a team of Community Health Workers CHWs and personally delivering CHW services in the field. This role is highly active and field based, with the manager spending the majority of their time conducting patient outreach, managing a caseload, and supporting Community Level Innovations CLI program participants, while also providing day to day coaching, workflow support, and oversight to other CHWs. This dual function position ensures consistent service delivery, staff support, and high quality patient engagement.

Requirements

  • Bachelor’s degree preferred
  • Community Health Worker certification required
  • At least 2 years of supervisory experience in non-profit, healthcare, or community based organization
  • Demonstrated experience conducting field-based outreach, case management, or direct community engagement
  • Bilingual in English and Spanish required
  • Strong written and verbal communication skills
  • Ability to build and maintain strong relationships with community partners
  • Ability to work occasional evenings or weekends for community events
  • Proficiency with electronic health record systems like Athena Health or eClinicalWorks
  • Must successfully pass credentialing process which includes verification of licenses and education, screening and possible vaccination for communicable diseases like Hepatitis B and TB

Responsibilities

  • Supervision of Community Health Worker Team (20%) Provide bi weekly one on one supervision and support to CHWs.
  • Monitor CHW caseloads and progress towards goals Lead training, workflow improvement, and performance support.
  • Manage schedules and field assignments to ensure efficient coverage and high quality service delivery.
  • Patient Outreach and Recruitment (50%) Maintain an active caseload and lead field based outreach efforts alongside the CHW team.
  • Develop and maintain relationships with partner organizations to support CLI program success.
  • Coordinate community events such as health fairs and resource fairs.
  • Administer the PRAPARE Screening Tool to identify patient needs and refer them to appropriate services at JFS and in the community.
  • Make appointments at the JFS Health Centers or JFS Food Pantry.
  • Patient Support and Education (25%) Provide case management support for assigned patients.
  • Serve as a liaison between CHWs and clinical providers.
  • Support CHWs with complex cases requiring higher-level problem-solving.
  • Facilitate or co lead health education classes.
  • Other Duties (5%) Analyze CLI program data for trends and quality improvement Track program expenses and manage gift card incentives.
  • Maintain detailed logs and prepare reports for agency leadership.
  • Other duties as assigned
