ABOUT THE ROLE Under the direction of the Community Health Director and consistent with the mission and policies of the YMCA of Western North Carolina, the Community Health Manager coordinates logistics essential to the success of the Chronic Disease Prevention and Management program, the department’s long-term objectives and ensures success of daily operations. This position supports data management, quality improvement practices, assists in routine administrative functions and program coordination and implementation as needed. This position is funded through the NC DHHS Office of Minority Health and is contingent upon continued grant support.

Requirements

  • Bachelor's degree in related field or 6 years’ experience working in related field in lieu of education.
  • 2 years professional experience in program development and leadership.
  • Excellent personal computer skills and experience with standard business software (Microsoft business suite).
  • Must have the ability to travel regularly and reliably throughout a three-county service area.
  • Lifestyle Coach credentialing or train to become a Lifestyle Coach within 12 months of hire date required.
  • Requires strong organizational skills, prioritization of workloads in order to meet deadlines, as well as detail-orientation and conscientiousness.
  • Must have excellent interpersonal, public relations, and communications skills; possess comfortability in both office and community-based work environments.

Nice To Haves

  • Health Coach or Community Health Work certification preferred.
  • Experience in working with historically marginalized populations preferred.
  • Bilingual in English/Spanish preferred.
  • 1+ year of experience working in priority population communities preferred.

Responsibilities

  • Responsible for coaching and mentoring direct reports and ensuring alignment with the organization’s personnel policy and HR practices within their scope of responsibility; provides input through quarterly check-ins and annual reviews with service line/department team.
  • Implements a results-based management approach and ensures service line/department is actively working towards association-level objectives by assessing progress towards identified goals/KPIs, provides timely communication, coaching and feedback to Lifestyle Coaches and discusses development opportunities.
  • Coordinates and implements community health programming and outreach initiatives to include launch and maintenance of chronic illness programs (DPP/MDPP/DEEP), data collection and management for all programs, quality assurance measures and accurate and timely reporting.
  • Oversees participant referral pipeline and supports potential program participants for enrollment in classes, supporting class placement, enrollment paperwork, database management and program evaluation.
  • Assists in receiving, tracking and responding to referrals from a variety of sources including DPP Express/ADA, medical providers, NCCARE360, website inquiries and other referral sources within a timely manner. Delegate needed action to the appropriate program staff.
  • Provides administrative oversight of incentive inventory, program supplies and other programmatic materials.
  • Develops and manages tracking tools/surveys to improve processes, program evaluation, program reporting and to inform budget development and forecasting.
  • With support of CH Director, supervises Lifestyle Coaches and coordinator staff, if applicable.
  • Assists in collaboration endeavors with community partners and experts to support health improvement efforts among low-income and marginalized populations, to include regular communication, marketing efforts around program promotion and increasing community support/partners.
  • Creates and distributes marketing materials as needed to ensure program success. May support digital marketing efforts.
  • Ensures program fidelity standards are in place and are followed by all staff. This includes the creation of Standard Operating Procedures (SOPs) to improve continuity and consistency of work.
  • Attends Community Health programs to support work and observe program quality. provide feedback to Director and Coaches, offering suggestions for process improvements.
  • Supports CH Director in budget creation and management for all chronic illness support programs, as needed.
  • Serves as a good steward of association resources.
  • Additional tasks as assigned.
