We specialize in serving patients with schizophrenia, bipolar disorder, substance use disorder, and related challenges—patients who are often overlooked by traditional healthcare systems. We provide advanced pharmacy services and wraparound care that ensure patients can access and stay on complex medications. We offer in-home and community-based injection support, help patients navigate insurance and prior authorizations, and work closely with providers to overcome barriers to treatment. In the Community Health Liaison (CHL) role, you will work under the supervision of the Community Health Liaison Manager. You will support AHCS Enhanced Clinical and Support Service Programs by working directly with behavioral health patients in clinic, parole, and home-based settings to improve medication adherence and overall health outcomes. You will also assist in coordinating care with clinical and pharmacy teams, providing patient support services, and identifying gaps in care to enhance treatment effectiveness. Success in this role will be demonstrated by improved patient engagement, adherence to treatment plans, and measurable optimization of health outcomes.
