As a Health Resources and Services Administration-designated health center operating under Section 330 of the Public Health Service Act, Tapestry serves as a critical safety-net provider, ensuring access to high-quality, equitable care regardless of a patient’s ability to pay. With a workforce of approximately 250 employees and an annual operating budget of $44 million, the organization is advancing a deliberate strategy focused on financial sustainability, operational rigor, and long-term community impact. Key priorities include strengthening core infrastructure, expanding value-based care arrangements to align reimbursement with outcomes, and optimizing participation in the 340B Drug Pricing Program to reinvest resources into patient care. Through this integrated approach, Tapestry 360 Health is positioned to sustain and scale mission-critical services for the communities it serves well into the future. Summary: The Community Health Coordinator promotes full access to Tapestry 360 Health services for specified populations on non-English-speaking patients and their families. They assist in language interpretation, care coordination, and bridging gaps in culture, communication and understanding between patients and healthcare providers. They will also support outreach and partnership activities with specified populations. This is a temporary, grant-funded position.
