About The Position

The Community Outreach Coordinator serves as a frontline liaison between LHS and the communities we serve. This position is responsible for serving as a link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. This role will serve as a bridge between the community and the health care, government, and social service systems. This person will build a trusting relationship with individuals and families in our community and will be a key member of our patient-centered health teams and community collaborators.

Requirements

  • High school diploma/GED or equivalent.
  • A valid driver’s license with a clean driving record.
  • Must have strong written and oral communication skills, including preparation and report presentation.
  • Knowledge of personal computers, PC applications, MS Word, Excel, PowerPoint and Project.
  • Effectively communicate with community members, partners and stakeholders.
  • Communicating health information using culturally appropriate terms and concepts.
  • Is self-motivated and detail oriented.
  • Problem solves with a human-centered approach.
  • Builds trust and collaboration with ease.

Nice To Haves

  • Community Health Worker certification preferred.
  • Experience with community health, public health or related field.
  • Experience with EPIC EHR preferred

Responsibilities

  • Provide direct outreach to patients to increase utilization of primary and preventative health care
  • Link patients to community resources and assist in navigating internal and external systems
  • Refer and link to preventative services through health screening and healthcare information
  • Enhance and expand clinic-based screenings, care coordination, and referral processes to connect internal programs and community-based initiatives to patients who demonstrate social needs and challenges
  • Engage in facilitating internal nutrition insecurity programs
  • Be a spokesperson/advocate for patients when needed
  • Bridge the gaps between healthcare and communities / social service systems
  • Increase access to care by providing culturally appropriate health information to patients, community members – including immunization education
  • Navigate individuals with the knowledge, skills and expanded resources to obtain healthcare at the right time and right place
  • Enhance LHS care delivery by aiding communication between healthcare providers, patients and community via enhanced cultural practices.
  • Align and promote health and well-being activities as part of infrastructure projects and with community sectors
  • Develop and maintain strong relationships with community partners, organizations and the key populations dedicated to serve
  • Serve as key point of contact for community partners related to collaborative health improvement strategies
  • Coordinate with the Marketing and Communications department to create meaningful messaging and marketing collateral to support health improvement initiative(s) for targeted population(s).
