The Community Health Coordinator supports initiatives that improve access to care, reduce social barriers to health, and assist vulnerable populations. This role collaborates with community partners and internal teams to advance health equity, connect individuals with resources, and help drive meaningful improvements in community well-being. Responsibilities

  • Develops and maintains a network of community resource centers, including development and implementation of curriculum, services and resource development for all locations.
  • Responsible for convening strategic partners and developing collaborations within the community to improve our program relations and expand our service locations.
  • Conducts ongoing needs assessment of surrounding community/neighborhoods to determine the most relevant topics for health education in the community resource centers.
  • Responsible for assisting in planning, coordinating and implementing family connection meetings in the community.
  • Facilitates recruitment and retention of participants in all program offerings.
  • Develop strategic community partnerships at agencies in the metro-area for our team members to have identified drop-in office space and meeting space in the community.
  • Plan, coordinate and implement education and access to resources at community resource centers.
  • Create and implement recruitment strategies for program participants.
  • Create and implement retention strategies for program participants.
  • Conduct regular resource needs assessments within the communities we serve.
