About The Position

Parkland Health Center has a new, grant-funded position created through the Rural Health Transformation Grant and represents the first step in advancing regional healthcare transformation efforts. The role will support one of the state's hubs and help lead initiatives across three regional hubs and counties, working with stakeholders to identify priorities and drive key community health improvement efforts. The position will be responsible for facilitating and coordinating transformation work, partnering with community organizations, behavioral health providers, ambulance districts, public health agencies, and other healthcare stakeholders. Ideal candidates will demonstrate professionalism, excellent communication skills, community-focused leadership, and experience or interest in public health and community collaboration.

Requirements

Responsibilities

  • Develop strategic community partnerships at agencies in the metro-area for our team members to have identified drop-in office space and meeting space in the community
  • Plan, coordinate and implement education and access to resources at community resource centers.
  • Create and implement recruitment strategies for program participants.
  • Create and implement retention strategies for program participants.
  • Conduct regular resource needs assessments within the communities we serve.
