Position Summary The Goal of the program is to remove barriers that our members face due to Social Determinants of Health issues, access to care and how to access needed services for their health care needs. This will be done in a linguistically, culturally appropriate manner. Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW’s help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This role requires face-to-face visits within Bronx, NY.

Requirements

  • 2 plus years experience providing clinical services
  • Experience in community/outpatient setting
  • Experience in Public Health
  • High School Diploma or equivalent GED
  • Experience using a computer including the use of Excel and Electronic Health Records

Nice To Haves

  • Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within
  • Health Care Condition knowledge like Diabetes, Hypertension, Cardiovascular Disease
  • Bachelors in Community Health and/ or Social Work
