About The Position

Requirements

  • High school diploma or equivalent required or TABE Level D (8th-grade reading & math)
  • Experience working in community health, social services, or a related environment.
  • Basic knowledge of public health principles and community outreach strategies.
  • Strong communication and interpersonal skills to engage effectively with diverse populations.
  • Ability to work independently and as part of a multidisciplinary team.
  • Resides in assigned community or sub region.
  • Able to provide year-round coverage of clinic hours.

Nice To Haves

  • Certification as a Community Health Worker (CHW) or similar credential.
  • Associate degree in health sciences or related field.
  • Experience working with underserved or vulnerable populations.
  • Proficiency in multiple languages commonly spoken in the community served.
  • Familiarity with electronic health records (EHR) and data collection tools.

Responsibilities
