The Community Health Advocate (CHA) supports community outreach, education, and relationship-building efforts that advance the hospice and serious illness care mission. Working under the direction of the Community Engagement Manager, the CHA assists with YMCA Resource Center activities, community education, and resource navigation, helping individuals and families access supportive services. Through relationship-based engagement, workshops, and community partnerships, the CHA addresses questions, reduces misconceptions, gathers community feedback, and strengthens awareness, access, and trust in hospice, palliative care, and related supportive resources.
