About The Position

The Community Habilitation Supervisor is responsible for overseeing the delivery of OPWDD Community Habilitation services within residential settings. This role ensures person-centered, high-quality supports are provided to individuals with developmental disabilities while supervising, training, and mentoring direct support staff. The position promotes compliance with OPWDD regulations, agency policies, and individualized Life Plans to support independence, health, and community integration.

Requirements

  • 1-2 years of related experience with people with intellectual and developmental disabilities.
  • 1-2 years of supervisory experience.
  • The candidate must be highly organized, detail-oriented, and demonstrate strong time management skills to effectively manage multiple responsibilities and priorities.
  • The candidate must be able to lead and collaborate effectively within a team environment, while also working independently when necessary to meet program needs and ensure quality service delivery.

Responsibilities

  • Supervise, support, and train staff to ensure consistent, person-centered service delivery in residential Community Habilitation programs
  • Attend OPWDD Life Plan meetings and collaborate with the interdisciplinary team to develop and implement Staff Action Plans aligned with individual goals and support needs.
  • Monitor the quality of Community Habilitation services provided in residential settings to ensure compliance with OPWDD regulations, agency standards, and individual service plans.
  • Complete formal staff evaluations, provide ongoing performance feedback, and offer coaching and mentoring to support staff development and retention.
  • Provide coverage in response to staff call-outs to ensure continuity of care and support for individuals receiving services.
  • Ensure accurate documentation of services, incidents, and progress notes in accordance with OPWDD and agency requirements.
  • Collaborate with residential leadership and clinical teams to support effective service coordination, health and safety, and individualized outcomes.
  • Assist in monitoring staffing patterns, unit utilization, and service delivery strategies to maintain program efficiency and quality standards.
  • Foster positive relationships with families, care managers, and external providers to support coordinated, person-centered care.
