About The Position

The Liaison II, Community Engagement Mentor will assist Day Program staff in effectively implementing Service Plans (SP) and program techniques through community inclusive activities. This is a full-time position eligible for Medical, Dental, Vision, Short Term Disability, Life/Accidental Death and Dismemberment, Flexible Spending Accounts, Long Term Disability, and several voluntary supplemental benefit offerings. In addition, these positions are eligible for paid time off in the form of vacation, sick, holiday, floating holiday, jury duty, and bereavement leave. All employees may also make pre-tax or post-tax (Roth) contributions to our retirement plan – must be 18 years of age to participate. This position is eligible for Daily Pay!

Requirements

• High School diploma or equivalent is preferred.
• Medication Administration Qualification (or complete training within 30 days).
• CPR/First Aid training or willingness to obtain training.
• Ability to interact, work cooperatively, and effectively.
• Must be able to respond to special requests and be flexible with regard to the job tasks and duties.
• Must have good physical stamina/endurance to stand for extended periods of time.
• Must demonstrate empathy and concern for individuals with developmental disabilities and have a passion for working to maximize their independence and personal growth.
• Must conduct self in a professional manner and maintain a professional appearance that enhances the agency’s reputation.
• Must be able to work effectively as a team member with the rest of the Day Program staff.
• Specific background requirements are required per contract.
• Must have the ability to drive for company business.
• Must be at least 25 years of age for all passenger transportation services.
• Must have a valid Colorado State driver license.
• Must be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.
• Must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition if using personal vehicle for company business.

Nice To Haves

• Previous experience with mild to severe developmentally disabled adolescents or adults is helpful.

Responsibilities

• Schedule community activities coinciding with curriculum, ensuring all activities are person-centered and related to individual goals.
• Provide transportation to clients in company vehicles and supervise their overall care while away from the facility.
• Collaborate with various community members to ensure client success in the community is fulfilling and safe.
• Assist the Day Program team in planning and scheduling reverse integration activities into the facility.
• Ensure proper, person-centered documentation is completed for each community integration activity, focusing on individual goals.
• Assist in the implementation of program plans, giving reinforcers, and providing one-on-one assistance and/or training.
• Assist in the preparation necessary for client arrival and departure and provide supervision at the beginning and end of the day to ensure safety of program participants when both exiting and entering the bus and facility.
• Communicate pertinent information to community program staff and assist program participants with transitioning throughout the day.
• Assist with medication administration when needed.
• Actively participate as a member of the Day Program team and assist team members with all client care.
• Participate in staff meetings as directed.
• Teach proper self-help skills and appropriate interactions through role modeling and programmatic intervention.
• Provide coverage in the programming areas and during lunch periods when at the facility.
• Fill out all necessary progress reports, per client, necessary for billing and tracking.
• Assist Case Managers with completing assessments by providing vital information needed on program participants.
• Maintain cleanliness of the vehicle’s interior and exterior.
• Complete daily inspections on the bus before driving it.
• Ensure the bus is in good maintenance at all times and schedule any maintenance for the bus as needed.
• Ensure all clients utilize seatbelts at all times, and assist clients on and off the bus at all times.
• Ensure the vehicle’s fuel tank is at least half-full at all times.
• Exhibit sound judgment when speaking with and assisting participants.
• Communicate in a timely manner and keep supervisor informed of progress, considerations, and pertinent information.
• Deal effectively and calmly with non-routine situations following accepted guidelines.
• Perform other duties as required.
• Communicate with parents, providers, or Host Home Providers, as assigned by Case Manager or supervisor.
