About The Position

SEEC is a progressive nonprofit agency providing a wide range of community supports to help people with developmental disabilities live the lives they choose. Supports are provided solely in the community and are directed by the interests, needs, and aspirations of each person. The Community Engagement Team assists individuals in their educational, recreational, fitness, and social endeavors with individualized support. They work with each person to create a monthly schedule of well-rounded, meaningful experiences that provide personal enrichment and prosperous employment. People receiving supports spend their entire day in the community and are involved in activities such as working, volunteering, or participating in job-readiness activities.

Requirements

  • Minimum of 1 year experience in supporting people to achieve personal goals or experience of comparable nature that has helped candidate build community-engagement skills
  • Strong analytical skills, good mentoring/coaching abilities and professional approach to personal development
  • Must be willing to learn and use technology such as cell phones, iPhone apps, photos, videos, laptops adaptative technology and case management database.
  • Must be willing to travel from multiple locations throughout Montgomery County as a floater.
  • Work in various employment, learning, and recreational environments including offices, retail stores, industrial locations, community arts and recreation facilities
  • Must be able to bend, reach, and lift a minimum of 50 lbs.

  • Using a personal vehicle is strongly preferred as they are following individual's daily schedules

Responsibilities

  • Implement Person Centered Plans which support people in achieving their outcomes and goals and in directing their own lives.
  • As part of implementing PCP, assist people in creating monthly schedules to include customized employment, learning opportunities, volunteering, recreational, and enrichment activities.
  • Work with program management to identify community resources.
  • Coordinate the schedules of multiple people on the caseload, and ensure schedules are followed daily. This will require flexibility and collaboration with colleagues.
  • Provide assistance with individual personal care needs.
  • Provide assistance with individual behavioral needs.
  • Provide assistance with individual safety needs while in the community.
  • Provide assistance with individual transportation needs (primarily through use of public transportation) including travel training (and learning how to facilitate it), booking trips, researching routes.
  • Provide daily documentation of people’s progress, schedule changes, activity schedules.
  • Communicate with team management, colleagues and with family and other caregivers, while also helping to work through any specific concerns or issues.
  • Strengthening connections between people with disabilities and community.
  • Attend trainings and ask questions related to implementing people’s goals and supporting individual needs.
  • Be inquisitive. Take initiative. Be open to learning about new tools and methods of support—such as adaptive technology, using iPhones and iPads.
  • Assist people in researching employment and recreational opportunities, completing applications/registrations and other job/activity/class-related internet or paperwork tasks, as necessary.
