About The Position

We are seeking a dedicated and compassionate Community Disability Practitioner (CDP) to provide person-centered support to individuals with diverse and complex needs. This role involves supporting individuals in their homes and communities while promoting independence, dignity, inclusion, and overall well-being. The successful candidate will work as part of a team of approximately 20 staff members and provide support to individuals with developmental disabilities, mental health concerns, and complex behavioral needs. This role supports individuals who require ongoing mental health, behavioral, and social support. Responsibilities may include: Supporting individuals with personal care, Managing situations involving physical and verbal aggression, Working with non-verbal individuals, Using communication software and American Sign Language (ASL), Assisting with home maintenance and cleaning tasks, Supporting activities of daily living, Attending medical appointments, Providing close supervision and ongoing monitoring to ensure health and safety.

Requirements

  • Valid driver's license and reliable driving ability
  • Ability and willingness to participate in swimming activities
  • Strong communication and interpersonal skills
  • Ability to work independently and collaboratively within a team environment

  • CPI (Crisis Prevention Intervention) Certification
  • Mental Health First Aid Certification
  • Suicide Intervention/Suicide First Aid Training
  • Experience supporting individuals with Autism Spectrum Disorder
  • Experience supporting individuals with schizophrenia
  • Experience working with seniors and older adults
  • Experience supporting individuals with Down Syndrome
  • Experience working with non-verbal individuals
  • Knowledge of ASL and alternative communication methods

Responsibilities

  • Provide mental health and social support to individuals with complex needs.
  • Assist with personal care and activities of daily living.
  • Support individuals with medical appointments and community participation.
  • Provide transportation and community access support.
  • Utilize Nonviolent Crisis Intervention (NVCI) techniques when required.
  • Support communication through sign language and communication software/tools.
  • Assist with household maintenance, cleaning, and daily routines.
  • Monitor health, safety, and well-being while maintaining professional and respectful relationships.
