Community Counselor III (Intensive Case Management)

Cascadia HealthClackamas, OR
$70,331 - $75,356Hybrid

About The Position

Cascadia Health delivers whole health care – integrated mental health and addiction services, primary care, and housing – to promote hope and support the well-being of the communities we serve. We envision a community where everyone benefits from whole health care, experiences well-being, and has a self-directed, connected life. The Intensive Case Management (ICM) program is a community-based, interdisciplinary team that provides flexible services tailored to each person’s changing needs and personal recovery goals. The length of services depends on each participant’s level of need and engagement. The team is made up of community counselors, skills trainers, and peer providers, and is expanding to collaborate with medical staff for a fully integrated approach. ICM is designed for individuals with serious mental illness (SMI) who benefit more from community-based services than traditional clinic-based care. The goal is to meet participants where they are, walking alongside them in their recovery journey, often using integrated dual disorder treatment to help them reach their goals. The Community Counselor III is responsible for providing and coordinating a wide range of supportive and therapeutic mental health services, while supporting a Trauma Informed Care environment. Services are provided from a Strengths Based philosophical framework, for a caseload of approximately twenty adults with mental illnesses that may not have successfully been served in more traditional clinic-based delivery systems. This team is moving towards a Dual Disorder Treatment (IDDT) approach. This is a dynamic team that uses creativity and 'out of the box' thinking to develop personal recovery plans one-on-one with clients, with the ultimate goal of helping clients overcome barriers to recovery.
