Job Purpose: The Community Connect Coordinator helps reduce social isolation by connecting people to their communities, building social capital, and strengthening individual and community capacity. This position will focus on improving parenting skills, health knowledge, and self-sufficiency through outreach, education, support, coaching, and advocacy. The incumbent will assist participants in accessing vital services like healthcare, economic assistance, and community-based resources.

Requirements

  • High School Degree or equivalent required.
  • Some experience in relationship-building and service coordination with both internal and external partners in a social services or community-facing organization required.
  • Some experience engaging in and assisting vulnerable families with children in their home setting required.
  • Must have current and working knowledge of local neighborhood and community services.
  • Ability to understand work with communities and families.
  • Excellent problem-solving skills
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Must be honest, dependable, and able to meet deadlines; Self-motivated and able to work independently.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
  • A valid Wisconsin Driver’s License or occupational driver’s license, reliable transportation and insurance is required.

Responsibilities

  • Works with staff and clients for the purpose of finding solutions and connecting with community partners to strengthen family well-being.
  • Collaborates with Wellpoint staff and families to address family issues related to the program model and access to community services.
  • Helps clients with scheduling, attending, and getting transportation to appointments with community agencies.
  • Works with families to identify and remove barriers to accessing services.
  • Assists families in obtaining knowledge and skills necessary to navigate the wealth of talents in their neighborhood, mobilize resources, and find solutions to their own problems.
  • Greets new Wellpoint Care clients by providing welcome materials.
  • Supports staff by making referrals to community agencies and resources, especially when families have complex needs.
  • Reviews staff requests for financial support from the family stability emergency fund, gathers additional information as needed, and makes recommendations to leadership.
  • Helps improve staff skills in assessing and documenting family needs in these Five Pillars of Stability for families served by Wellpoint Care.
  • Conducts ongoing assessments of program needs based on program model used.
  • Participates in community outreach activities to raise awareness about Wellpoint programs.
  • Engages the community through outreach to promote child safety and help prevent child abuse and neglect.
  • Maintains strong partnerships with community organizations to meet the ongoing needs of families served by Wellpoint Care.
  • Seeks new partnerships based on the needs of children and families in nearby neighborhoods.
  • Advocates clearly and effectively for the needs of families receiving services through Wellpoint Care.
  • Builds and maintains internal partnerships with Wellpoint staff by regularly communicating with service staff and leadership, and attending internal meetings.
  • Shows the ability to maintain productive and respectful relationships with team members and community partners.
  • Provides support, assistance, and training to staff on program models and client engagement.
  • Takes an active role in Agency and program development activities to help improve the quality of services offered by Wellpoint and enhance Agency growth and community partnerships.
  • Supports Supervisor in the development and implementation of additional supportive services for Wellpoint Care families, as well as maintain the volunteer pool (and Agency database) in an effort to help support more community-based partnerships.
  • Maintains the Wellpoint resource room and ensures that clients and Wellpoint employees have timely access to goods that meet their basic needs (i.e., Clothes, non-perishable foods, bedding, winter gear, etc.).
  • Develops and maintains internal resource directory of community partners and resources that is relevant and up to date for all staff that support families.
  • Informs Wellpoint Care teams about new or changing resources.
  • Enters program model data into Agency database and collaborates with Quality Improvement to analyze the data.
  • Supports the facilitation of required documentation and data entry related to volunteers.
