The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) is seeking a qualified and experienced individual for the position of Community Clinician to join their team at the Southwest Connecticut Mental Health System (SWCMHS) in Bridgeport, CT. This full-time, 40-hour, 1st shift position (Monday - Friday, 8:00 am - 4:30 pm) plays a key role in delivering direct clinical and case management services to adults experiencing complex behavioral health and substance use challenges. The role aims to promote client safety, stability, and recovery within community settings. As part of a multidisciplinary team, the clinician will provide trauma-informed, person-centered, and recovery-focused support to help individuals achieve their treatment goals. Job duties include implementing individualized treatment plans, providing ongoing support, delivering case management services, coordinating care, connecting clients with community resources, conducting supervised intake assessments and evaluations, participating in counseling sessions, monitoring client progress (including medication compliance), collaborating with other providers, providing crisis intervention, outreach, referrals, and offering education and training in daily living skills.
