About The Position

The State of Connecticut Department of Mental Health and Addiction Services (DMHAS) is seeking a qualified and experienced individual for the position of Community Clinician to join their team at the Southwest Connecticut Mental Health System (SWCMHS) in Stamford, CT. This is a full-time, 40-hour position on the 1st Shift, Monday - Friday, 8:00 am - 4:30 pm. The Community Clinician plays a key role in delivering direct clinical and case management services to adults experiencing complex behavioral health and substance use challenges. This position helps promote client safety, stability, and recovery within community settings. As a member of a multidisciplinary team, the clinician will provide trauma-informed, person-centered, and recovery-focused support to help individuals achieve their treatment goals. Job duties include implementing individualized treatment plans, providing ongoing support to clients on caseload, delivering case management services, coordinating care, connecting clients with community resources, conducting supervised intake assessments, evaluations, participating in counseling sessions, monitoring client progress, collaborating with other providers, providing crisis intervention, outreach, referrals, and offering education and training in daily living skills.

Requirements

  • A Bachelor's degree in a health care field and two (2) years of experience working independently using clinical case management techniques in a behavioral health setting.
  • Knowledge of behavioral health counseling techniques; social work methodology, casework, group work and community mobilization; family dynamics and dynamics of interpersonal relationships; social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of clients and families; community resources available to individuals and families; approaches to treatment; psychotropic and addiction medications.
  • Considerable interpersonal skills.
  • Oral and written communication skills.
  • Considerable ability to prepare clear and concise reports and case histories.

Responsibilities

  • Implement individualized treatment plans and provide ongoing support to clients on caseload.
  • Deliver case management services, including coordinating care and connecting clients with community resources.
  • Conduct supervised intake assessments, evaluations, and participate in counseling sessions.
  • Monitor client progress, including medication compliance, and collaborate with psychiatrists, nurses, and other providers.
  • Provide crisis intervention, outreach, and referrals to appropriate services when needed.
  • Offer education and training in daily living skills to promote client independence.
  • Functions primarily to implement treatment plans for clients on caseload.
  • Provides case management services to clients.
  • Assists clients in linking with community support systems and agencies.
  • Monitors clients progress within mental health and community support systems.
  • Performs intake, evaluation and assessment of treatment programs under supervision of a licensed independent practitioner.
  • Participates in counseling sessions.
  • Provides education and training to clients in community living and activities of daily living skills.
  • Monitors clients medication compliance and coordinates meetings with team psychiatrists or nurses as necessary.
  • Provides appropriate interventions to clients in crisis including outreach, counseling and/or referral to other service providers.
  • Completes all required documentation related to client treatment in accordance with established procedures.
  • May transport clients to and from appointments.
  • May assist in development of treatment plans for new clients.
  • May administer medication.
  • May provide nursing consultation and/or input.
  • Performs related duties as required.
