The Community Case Manager works directly with individuals receiving services, connecting them with resources, documenting client progress and supporting follow-through on treatment plans and referrals. Requirements

• An Associate’s Degree and two years experience and/or Bachelor’s Degree
• The ability to establish and maintain supportive relationships with children, youth and families
• Knowledge on how to provide holistic and therapeutic case management services
• Experience working in the mental health field
