About The Position

LCMHA (Lenawee Community Mental Health Authority) provides or manages a full array of services to adults and children with mental health and/or emotional disturbances, persons with substance use disorders, and persons with developmental disabilities. Our mission is to create a path to resilience, recovery, wellness, and self-determination for these individuals. The Case Manager Assistant – Home-Based (Children's Services) assists in meeting the needs of consumers receiving case management services. Specifically, this individual will provide supports and services to a consumer as identified in the child's Individual Plan of Service. This role performs duties under the guidance of the Home-Based clinician, which may include assisting and training in family implementation of behavioral interventions, assisting families in practicing pro-social behavior, supporting parents in implementing parenting strategies identified in the plan, modeling positive parent/child interactions, and application for benefits. The position also involves maintaining documentation of contacts with consumers, assisting the team in maintaining the calendar of consumer contacts, and participating in Home-Based team meetings and huddles to ensure cohesive interventions with families served. Other assistance to families as delegated by the Home-Based team, regular and consistent attendance as assigned, and carrying out additional duties as assigned by supervisor are also part of the role.

Requirements

  • High school diploma or equivalent is required
  • Possess a valid U.S. driver's license, with an acceptable driving record
  • Basic computer knowledge (such as familiarity with email and internet use) and experience with Microsoft Office applications (Word, Excel, Power Point, etc.) is highly desirable.

  • Previous experience working with children with serious emotional disturbances is desired.

Responsibilities

  • Provide supports and services to a consumer as identified in the child's Individual Plan of Service
