About The Position
The Community Case Manager works directly with individuals receiving services, connecting them with resources, documenting client progress and supporting follow-through on treatment plans and referrals. This role is crucial in transforming the lives of those at risk, aiming for significant and measurable improvement in clinical outcomes.
Requirements
- An Associate’s Degree and two years experience and/or Bachelor’s Degree.
- The ability to establish and maintain supportive relationships with children, youth and families.
- Knowledge on how to provide holistic and therapeutic case management services.
- Experience working in the mental health field.
