About The Position

The Case Manager will coordinate and deliver integrated services to children and families within the UCS Children’s Integrated Services program. This role focuses on early intervention and mental health support for children from birth to age 5 and their families, helping them navigate social, emotional, and behavioral challenges.

Requirements

  • High school diploma required.
  • 2-4 years of relevant experience working with preschool-aged children, such as in roles like preschool teachers or aides, or in case management with children and families.
  • Valid driver’s license and auto insurance.

Nice To Haves

  • Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred.

Responsibilities

  • Conduct comprehensive assessments of children and families, developing individualized service plans (One Plans) tailored to their unique needs.
  • Facilitate and monitor services across various providers, ensuring families have access to the necessary resources and support systems.
  • Provide education and guidance to families, helping them build advocacy skills and navigate community services.
  • Work with multidisciplinary teams, including educators, healthcare providers, and community organizations, to ensure effective service delivery.
  • Attend weekly, monthly, and quarterly meetings for the CIS program to report progress and track data for grant stakeholders.
  • Collect and maintain data related to client outcomes and service delivery to provide comprehensive reports to stakeholders.
  • Work within homes and classrooms to provide support to caregivers and educators, helping them accommodate and manage children's behaviors effectively.
  • Regularly assess client progress, adjusting service plans as needed to respond to changing family dynamics.
  • Maintain accurate and timely records of client interactions and services in compliance with UCS contract requirements.
  • Engage in outreach activities to raise awareness about UCS services, promoting access for families in need.
