Community Care Coordinator (Home Health & Hospice)
About The Position
This role is more than a marketing position; it's a platform for professional growth. As a Home Health & Hospice Community Liaison at Billet Health, you will build and strengthen relationships within the Las Vegas healthcare community. You will represent a growing organization with an integrated continuum of care, including Home Health, Hospice, Pharmacy, Mobile Providers, and Palliative Care. This integrated approach allows for extending your network across multiple care areas, fostering stronger partnerships, supporting patients and families, and expanding your professional impact. The role offers opportunities to earn bonuses through hospice referrals, home health opportunities, and additional bonus opportunities through pharmacy and other services. You will expand relationships across multiple healthcare settings and build long-term partnerships within your market.
Requirements
- 1+ years of Home Health or Hospice marketing experience, or proven healthcare sales experience
- Established referral relationships within the Las Vegas market
- Knowledge of Medicare guidelines and healthcare compliance
- Excellent communication and relationship-building skills
- Ability to work independently and manage a territory
- Strong organizational and follow-up skills
- Valid driver's license
- Reliable transportation
- Current auto insurance
Responsibilities
- Build and maintain relationships with physicians, hospitals, SNFs, ALFs, case managers, and community organizations
- Develop and execute territory growth strategies
- Conduct face-to-face outreach and educational presentations
- Build new referral relationships while strengthening existing partnerships
- Represent Billet Health at community events and professional networking opportunities
- Meet with patients and families to provide information about Home Health and Hospice services
- Collaborate with clinical teams to support smooth patient transitions
- Track outreach, referrals, leads, and outcomes
- Identify market opportunities and communicate them to leadership
- Contribute to monthly growth objectives
