About The Position

Assisting Hands Home Care Cary is seeking an experienced, compassionate, and highly organized Community Care Coordinator to join their growing team. This role is ideal for a CNA or experienced home care professional with hands-on caregiving experience who is ready to transition into a coordination and leadership position. The Community Care Coordinator will be responsible for overseeing care delivery across multiple senior living communities in Cary and Raleigh, ensuring residents receive reliable, compassionate, and personalized care. This role also involves supporting caregivers, families, and the office team. The Community Care program differs from traditional one-on-one home care, as caregivers may assist multiple residents daily, requiring strong organization, communication, time management, and leadership skills for success.

Requirements

  • CNA or significant hands-on home care experience.
  • Understanding of the daily challenges of working with older adults and their families.
  • Experience providing personal care and assisting with activities of daily living.
  • Comfort working with clients who may have dementia, confusion, behavioral challenges, or changing care needs.
  • Ability to confidently handle difficult conversations and challenging situations.
  • Comfort addressing concerns with caregivers and holding team members accountable.
  • Clear, professional, and consistent communication skills.
  • Highly organized with the ability to manage multiple locations and priorities.
  • Ability to follow through and ensure issues are resolved.
  • Strong problem-solving skills and ability to think independently.
  • Comfort training and coaching caregivers.
  • Ability to build positive relationships with clients, families, caregivers, and senior living community staff.
  • Understanding of the importance of documenting and communicating changes promptly.
  • Reliable, dependable, and able to work independently while remaining connected to the office team.
  • Valid driver's license and reliable transportation to travel between Cary and Raleigh locations.

Responsibilities

  • Visit and regularly check in at multiple senior living communities in Cary and Raleigh.
  • Check in with caregivers and residents to ensure scheduled services are being completed.
  • Follow up with clients and families to ensure they are satisfied with the care they are receiving.
  • Communicate clearly and professionally with residents, families, caregivers, community staff, and the Assisting Hands office.
  • Identify and address concerns quickly and professionally.
  • Handle challenging situations involving clients, caregivers, and family members with patience, professionalism, and confidence.
  • Provide ongoing training, coaching, and support to caregivers.
  • Train new caregivers on Community Care procedures, expectations, and client-specific tasks.
  • Ensure caregivers understand and follow detailed care schedules and task lists.
  • Monitor caregiver performance and provide feedback when needed.
  • Communicate changes in client needs or care plans to the office.
  • Help coordinate caregiver schedules and coverage when needed.
  • Follow up on missed tasks, schedule changes, and client concerns.
  • Ensure medication reminder protocols and other Community Care procedures are being followed.
  • Maintain strong relationships with senior living community staff and leadership.
  • Keep the Assisting Hands office informed of what is happening in each community.
  • Attend weekly office meetings and provide updates on each location.
  • Assist with improving systems and processes as the Community Care program continues to grow.
