About The Position

Integrated Services for Behavioral Health (ISBH) is seeking a Community Behavioral Health Worker to join their team in Fairfield County, OH. ISBH is a community-minded, forward-thinking behavioral health organization dedicated to helping individuals and families on their road to health and well-being. They provide services in homes and communities, connecting individuals with necessary resources. Serving Southeastern and Central Ohio, ISBH offers a comprehensive array of behavioral health and other services, collaborating with local partners to foster healthy people and strong communities. The Community Behavioral Health Worker will provide direct services aimed at empowering individuals and families to build skills and overcome barriers to behavioral health and daily functioning. This role involves coaching and practical support in home, school, and community settings, assisting clients in developing routines, coping strategies, and healthy communication. The CBHW will also guide clients in accessing community resources, collaborate with internal and external teams, and maintain strong engagement with clients and caregivers. All services are delivered using trauma-informed and strength-based approaches to promote recovery and resilience. The pay range for this position is $19.00 to $22.26 an hour, based on experience, education, and/or licensure.

Requirements

  • A High School diploma or GED equivalent is required
  • A bachelor’s degree in related field or minimum 3 years of direct relevant experience is required
  • Experience with multi-need individuals and families is required
  • Broad knowledge of community service systems is required
  • Advanced communication skills, both oral and written is required
  • Advanced organizational skills are required
  • Ability to bill Medicaid without restrictions is required
  • A valid driver's license and the ability to operate a motor vehicle is required
  • An appropriate level of auto insurance coverage is required
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required
  • Ability to maintain confidential information is required
  • Ability to manage deadlines within a fast-paced, high-volume environment is required
  • Ability to operate in an Internet-based, automated office environment is required
  • Ability to maintain high-speed internet connection is required
  • Prolonged periods of sitting at a desk and working on a computer is required
  • Must be able to lift up to 15 pounds at times is required

Nice To Haves

  • Experience partnering with local behavioral health systems, housing, legal systems, and social services is preferred
  • 3+ years of experience in related field is preferred

Responsibilities

  • Engage individuals and families in the home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning
  • Provide skill building, coaching, and practical supports in home, school, and community settings
  • Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns
  • Assist clients in navigating community resources through modeling, coaching and teaching
  • Deliver care in home and community-based settings
  • Collaborate with internal programs and external partners to ensure quality of care
  • Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through
  • Utilizes databases and Electronic Health Record systems for accurate client documentation
  • Adhere to ISBH EHR documentation policy
  • Meet ISBH standards for productivity and quality of service delivery
  • Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
  • Participate in team meetings, supervision, and required training to support ongoing development
  • Use trauma-informed, strengths-based approaches in all client and team interactions
  • Travel within the designated service areas
  • Perform other duties as assigned
