About The Position

NeighborHealth, formerly East Boston Neighborhood Health Center, is a large community health center serving the greater Boston area with a commitment to patient and community health. This role is within the Project Shine department and is a full-time position. The Community Based Prevention Health Coordinator reports to the Project SHINE Department Manager and collaborates with the Community Based Prevention Program educator and Prevention and Screening team. The position involves in-person and virtual outreach, risk assessment, risk-reduction education, implementation of CDC evidence-based Personalized Cognitive Counseling (PCC) curriculum, and linking patients to PrEP and behavioral health services. A key focus is leading CBP HIV and STDs prevention initiatives, particularly within the Latinx gay, bisexual, and other men who have sex with men community. Bilingual (English/Spanish) skills are required. The role also involves leading the PCC program, connecting participants to comprehensive behavioral health services, providing individual risk assessments, facilitating PrEP nights/socials, and ensuring programmatic and evaluation compliance with funders.

Requirements

  • Bachelor’s Degree in either Arts or Science Required
  • Minimum of 1 year relevant experience, preferably in a community health, mental health or hospital setting.
  • Excellent working knowledge of public health paradigms and evidence-based interventions for at risk populations.
  • Ability to work through brief patient contacts, and make quick and accurate clinical assessments of mental and behavioral health conditions.
  • Comfortable with the pace of primary care and emergency care, within with an interdisciplinary team.
  • Willingness to work with the guidelines within assigned behavioral interventions, such as Personal Cognitive Counseling and more.
  • Ability to work independently and collaboratively, and demonstrate sound judgment.
  • Willing to provide feedback and receive feedback.

Nice To Haves

  • Master’s Degree in related field (public health, health and/or human services, social work related) preferred.

Responsibilities

  • Under the direction of the Director, leads the PCC program to accomplish its objectives and goals.
  • Works collaboratively with NeighborHealth's Marketing Department and helps coordinate marketing efforts.
  • Responsible for data collection, reports, monitoring and evaluation of program.
  • Works closely with NeighborHealth's Grants Department to ensure appropriate and timely expenditure tracking, and grant compliance.
  • Ensures the goals of the PCC program are met on a consistent basis.
  • The provision of direct patient care, including assessment, supported referrals to PrEP services and behavioral health services.
  • Follow up and oversight for special high-risk populations.
  • This includes coordination of services with outside behavioral health providers when necessary and periodic outreach to engage patients in care.
  • Ensures program documentation, reporting, evaluation and overall compliance. Maintain confidentiality of patient records.
  • Develop and sustain relationships with local agencies in East Boston.
  • Ability to work collaboratively with multiple community and city-wide stakeholders.
