Community Empowerment Resources (CER) is seeking a compassionate, motivated Community-Based Case Manager to join their growing team. This is a field-based position focused on recovery-oriented, trauma-informed care delivered directly in the community. CER is a mission-driven nonprofit behavioral health organization providing comprehensive services to individuals experiencing serious and persistent mental illness (SPMI), co-occurring substance use disorders, and housing instability. As a Case Manager, you will build clinically informed relationships with adults diagnosed with SPMI and support them in achieving stability, independence, and improved quality of life through individualized recovery planning and coordinated services. Caseloads are intentionally kept low (approximately 10–12 clients) to ensure meaningful engagement and high-quality care.
